This role includes assessment, care planning, care coordination, discharge follow-up, patient and caregiver education, and collaboration across clinical, social work, and community resources. /ppbRequired Qualifications: /b /pullibEducation: /bAssociates Degree of Nursing. Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), or the Commission on Collegiate Nursing Education (CCNE) /lilibCertifications: /bulliBasic Life Support (BLS) /liliPossess one of the following certifications: /liliCommission for Case Manager Certification Certified Case Manager (CCM) Certification of Disability Management Specialists Commission:ulliCertified Disability Management Specialist (CDMS)



/li /ul /liliAssociation of Rehabilitation Nurses:ulliCertified Rehabilitation Registered Nurse (CRRN) /liliAmerican Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S). /li /ul /liliNational Board for Certification in Continuity of Care:ulliAdvanced Certification in Continuity of Care (ACCC) /li /ul /liliCommission on Rehabilitation Counselor Certification:ulliCertified Rehabilitation Counselor (CRC) /liliAmerican Nurses Credentialing Center Nurse Case Manager (RN-NCM) /li /ul /liliNational Academy of Certified Care Managers:ulliCare Manager Certified (CMC) /li /ul /li /ul /lilibLicense: /bCurrent, full, active, and unrestricted license to practice as a Registered Nurse. /lilibExperience: /bMinimum of two (2) years recent experience within the last four (4) years in professional nursing or Case Management. Under Utilization requires six (6) years of broad-based clinical nursing experience and previous experience in UM is preferred. /li /ulpU.S. Citizenship is required /ppbKey Responsibilities: /b /pulliParticipate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care. /liliProvide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring.



/liliIdentify opportunities for CM and identify and integrate local CM processes. /liliDevelop and implement local strategies using inpatient, outpatient, onsite and telephonic CM /liliDevelop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources. /liliIntegrate CM and utilization management (UM) and integrating nursing case management with social work case management. /liliMaintain liaison with appropriate community agencies and organizations. Accurately collect and document patient care data. /liliDevelop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness. /liliEstablish mechanisms to ensure proper implementation of patient treatment plan and follow up post discharge in ambulatory and community health care settings. /liliProvide appropriate health care instruction to patient and/or caregivers based on identified learning needs. /li /ulpbPhysical requirements: /bAbility to sit or stand for extended periods while reviewing medical records and working on a computer. Frequent use of standard office equipment and use of clear verbal and written communication.
