Primary Care Nurse Practitioner Position Type: Full-time, Permanent Location: Beachburg, Cobden & Eganville, Renfrew County, ON Term: 1.0 FTE, ongoing Salary Range: $114,200 to $142,700 (1.0 FTE) Position Summary The Primary Care Nurse Practitioner (NP) functions as a member of the Primary Care Team in the provision of primary health care services offered at Connect Well Community Health. Primary health care services are provided at Connect Well and in the community, with an emphasis on health promotion, client education, and self‐management. The RN (EC) must be aware of, understand, and adhere to the Standards of Practice set out by the College of Nurses of Ontario.
Responsibilities Provide primary health care to clients at Connect Well Community Health and in the community. Offer health promotion and client education to support self‐management. Work effectively within an inter‐disciplinary team. Provide on‐call telephone services on a rotation basis. Support evening hours as required.
Qualifications Completion of a recognized Primary Care Nurse Practitioner program. Master's degree in Nursing, or equivalent recognized education and experience. Current and unrestricted registration as an RN (EC) with the College of Nurses of Ontario.
Two years of experience in the nurse practitioner role in a primary care setting (community experience preferred). Demonstrated ability to work effectively in an inter‐disciplinary team.
Demonstrated leadership in promoting a shared responsibility for professional practice and development.
Ability to work evening hours, as required. Ability to provide on‐call telephone services on a rotation basis. Valid driver's licence and use of a vehicle.
