ConnectWell Community Health is seeking a Primary Care Nurse Practitioner to join the Primary Care Team. The NP delivers primary health care at ConnectWell and in the community, with emphasis on health promotion, client education, and self‐management.
Requirements include completion of a recognized NP program, a Master's degree in Nursing or equivalent, current RN (EC) registration with the College of Nurses of Ontario, and two years' NP experience in primary care.
