/p pbJob Summary: /bThe Registered Nurse - Case Manager (CM), Utilization Manager (UM) is responsible for coordinating patient care, overseeing case management activities, and supporting utilization management to ensure appropriate, efficient, and high-quality healthcare delivery.
This role includes assessment, care planning, care coordination, discharge follow-up, patient and caregiver education, and collaboration across clinical, social work, and community resources.
/p pbRequired Qualifications: /b /p ul libEducation: /bAssociates Degree of Nursing.
Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), or the Commission on Collegiate Nursing Education (CCNE) /li /ul pbCertifications: /b /p ul liBasic Life Support (BLS) /li liPossess one of the following certifications: /li liCommission for Case Manager Certification Certified Case Manager (CCM) Certification of Disability Management Specialists Commission: /li liCertified Disability Management Specialist (CDMS) /li liAssociation of Rehabilitation Nurses: /li liCertified Rehabilitation Registered Nurse (CRRN) /li liAmerican Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN)

/li liNational Board for Certification in Continuity of Care: /li liAdvanced Certification in Continuity of Care (ACCC) /li liCommission on Rehabilitation Counselor Certification: /li liCertified Rehabilitation Counselor (CRC) /li liAmerican Nurses Credentialing Center Nurse Case Manager (RN-NCM) /li liNational Academy of Certified Care Managers: /li liCare Manager Certified (CMC) /li /ul pbLicense: /bCurrent, full, active, and unrestricted license to practice as a Registered Nurse.
/p pbExperience: /bMinimum of two (2) years recent experience within the last four (4) years in professional nursing or Case Management.
Under Utilization requires six (6) years of broad-based clinical nursing experience and previous experience in UM is preferred.

Citizenship is required /p pbKey Responsibilities: /b /p ul liParticipate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care.
/li liProvide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring.
/li liIdentify opportunities for CM and identify and integrate local CM processes.
/li liDevelop and implement local strategies using inpatient, outpatient,



onsite and telephonic CM /li liDevelop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources.
/li liIntegrate CM and utilization management (UM) and integrating nursing case management with social work case management.
/li liMaintain liaison with appropriate community agencies and organizations.
Accurately collect and document patient care data.
/li liDevelop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
/li liEstablish mechanisms to ensure proper implementation of patient treatment plan and follow up post discharge in ambulatory and community health care settings.
/li liProvide appropriate health care instruction to patient and/or caregivers based on identified learning needs.
/li /ul pbWork Environment Schedule: /bMonday - Friday; 8 hour shifts; Aviano Airbase, Italy /p pbPhysical requirements: /bAbility to sit or stand for extended periods while reviewing medical records and working on a computer.
Frequent use of standard office equipment and use of clear verbal and written communication.
Must have sufficient visual acuity and attention to detail to analyze complex clinical documentation, with occasional ability to move about the workplace and lift up to ***** pounds.
