Corrections Victoria is seeking an Assessment and Transition Co-ordinator (ATC) at Western Plains Correctional Centre to join the Offender Services team. The role focuses on coordinating reintegration pathways, transition and discharge planning, and referrals to pre- and post-release services within a prison setting.

You will work with multi-disciplinary teams to develop integrated care plans, engage prisoners in goal setting and program participation, and ensure compliance with service delivery
