Job Description Work Schedule: 3 x 12 hour shifts, rotating weekends, 7pm-7am
Sign on Bonus: $15,000
- Identifies and formulates expected outcomes based on diagnoses, ensuring they are realistic, attainable, and culturally appropriate.
- Develops individualized plans of care in collaboration with the patient, family, and healthcare providers, ensuring continuity and documentation.
- Implements interventions from the plan of care safely, timely, and appropriately, documenting actions taken.
- Evaluates patient progress towards outcomes, revising diagnoses, outcomes, and plans of care as needed.
- Administers medications and treatments as prescribed, ensuring patient safety and adherence to protocols.
- Communicates effectively with patients, families, physicians, and staff, maintaining professional relationships and promoting a positive environment. Educates patients and families on health conditions, procedures, and car
