Job Summary:

The Registered Nurse - Case Manager (CM), Utilization Manager (UM) is responsible for coordinating patient care, overseeing case management activities, and supporting utilization management to ensure appropriate, efficient, and high-quality healthcare delivery. This role includes assessment, care planning, care coordination, discharge follow-up, patient and caregiver education, and collaboration across clinical, social work, and community resources.





Required Qualifications:

Education:

Associates Degree of Nursing. Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), or the Commission on Collegiate Nursing Education (CCNE)

Certifications:

Basic Life Support (BLS)

Possess one of the following certifications:

Commission for Case Manager Certification Certified Case Manager (CCM) Certification of Disability Management Specialists Commission:

Certified Disability Management Specialist (CDMS)

Association of Rehabilitation Nurses:

Certified Rehabilitation Registered Nurse (CRRN)

American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S).

National Board for Certification in Continuity of Care:

Advanced Certification in Continuity of Care (ACCC)

Commission on Rehabilitation Counselor Certif
