Job Summary: The Registered Nurse - Case Manager (CM), Utilization Manager (UM) is responsible for coordinating patient care, overseeing case management activities, and supporting utilization management to ensure appropriate, efficient, and high-quality healthcare delivery. This role includes assessment, care planning, care coordination,
discharge follow-up, patient and caregiver education, and collaboration across clinical, social work, and community resources.
Required Qualifications:
- Education: Associates Degree of Nursing. Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), or the Commission on Collegiate Nursing Education (CCNE)
- Certifications:
- Basic Life Support (BLS)
- Possess one of the following certifications:
- Commission for Case Manager Certification Certified Case Manager (CCM) Certification of Disability Management Specialists Commission:
- Certified Disability Management Specialist (CDMS)
- Association of Rehabilitation Nurses:
- Certified Rehabilitation Registered Nurse (CRRN)
- American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S).
