CenterWell Home Health seeks a licensed Physical Therapist Assistant to provide in-home care, following the Physical Therapist's plan to improve function and independence. You will deliver manual therapy, modalities, and document progress for each visit.
Responsibilities include caring for geriatric patients, coordinating with the Physical Therapist and branch leadership, and ensuring safety and infection control during care delivery. Driving to patient homes is required, with full-time hours.
