hospital, etc.) based on individual needs. The Hospice RN Case Manager is responsible for independent management of the Hospice patient population requiring use of advanced assessment, teaching and decision-making skills. The Hospice RN Case Manager cares for a caseload of hospice patients by evaluating the patient for appropriateness of hospice care and developing the plan of care in collaboration with the provider, caregivers and Hospice IDG members. The Hospice RN Case Manager provides follow up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers response to the plan. The Hospice RN Case Manger provides patient care based on provider’s orders, departmental policies and procedures, regulatory guidelines and best practice.
