As a Hospice Nurse Case Manager, you will provide skilled nursing care, coordinate interdisciplinary services, and support patients and caregivers through every stage of the hospice journey.
Position Summary
The Hospice Nurse Case Manager is responsible for managing and coordinating comprehensive hospice care for assigned patients. This role includes conducting assessments, developing individualized care plans,
providing direct patient care in the home, collaborating with the interdisciplinary team, and serving as a clinical resource to patients, families, and caregivers.
Key Responsibilities
Patient Care & Case Management
Conduct admission, pre-admission, routine, urgent, and pronouncement visits.
Perform comprehensive physical, psychosocial, and environmental assessments.
Develop, implement, and revise individualized patient care plans.
Provide skilled nursing care and symptom management in accordance with hospice standards.
Respond to patient needs, medical crises, and family concerns in a timely manner.
Supervise home health aides and collaborate with caregivers to ensure quality patient care.
Maintain accurate and timely clinical documentation.
Patient & Family Education
Educate patients, families, and caregivers on disease progression, symptom management, medications, and end-of-life care.
Teach caregivers how to safely perf
