What You’ll Do
As a Care Coordinator, you will oversee and manage the entire member journey from onboarding through diagnostics, plan development, care implementation, and renewal planning. You will ensure each step is personalized, accurate, and timely. You will serve as the primary point of contact for your assigned members, fostering trust through effective communication, scheduling,
proactive outreach, and ongoing support. Your daily efforts have a significant impact on both clinical quality and emotional engagement.
You will collaborate with multiple teams to ensure care plans are executed and followed up on. You will track milestones, identify gaps, and promote teamwork across departments. From verifying diagnostic readiness to coordinating plan meetings and preparing mid-year reviews, you will ensure that every member’s experience reflects the Fountain Life standard of accuracy and excellence.
