/p ul liPT Licensure - Michigan /li liSign-On Bonus $**** /li liFleet Car Program Available /li liFull Benefits (Medical, Dental, Vision) with 401k match /li /ul h3What You'll Do as a Home Health Physical Therapist /h3 ul liDeliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
/li liEvaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
/li liAdjust treatment plans and techniques as needed to achieve optimal outcomes.
/li liCollaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.
/li liEducate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
/li liDocument all visits, assessments, and progress notes accurately and in a timely manner.
/li liSupport patients in maximizing their independence and improving quality of life—wherever they call home.
/li /ul h3About You /h3 h3Qualifications – What You'll Bring /h3 ul liMaster's degree in Physical Therapy from an accredited program.
/li liCurrent PT license in the state of employment (or eligibility to obtain).
/li liCPR certification required.
/li liStrong communication, assessment, and problem-solving skills.
/li liAbility to work independently in a variety of home environments.
/li liValid driver's license, auto insurance, and reliable transportation.
