Hospice Nurse Case Manager (RN/LPN) – Home Hospice

Schedule: Full-time, M- F 8am-5pm with occasional on-call.

As a Hospice Nurse Case Manager, you will provide skilled nursing care, coordinate interdisciplinary services, and support patients and caregivers through every stage of the hospice journey.

Position Summary The Hospice Nurse Case Manager is responsible for managing and coordinating comprehensive hospice care for assigned patients. This role includes conducting assessments, developing individualized care plans, providing direct patient care in the home, collaborating with the interdisciplinary team, and serving as a clinical resource to patients, families, and caregivers.

Key Responsibilities

Patient Care & Case Management

Conduct admission, pre-admission, routine, urgent, and pronouncement visits.

Perform comprehensive physical, psychosocial, and environmental assessments.

Develop, implement, and revise individualized patient care plans.

Provide skilled nursing care and symptom management in accordance with hospice standards.

Respond to patient needs, medical crises, and family concerns in a timely manner.

Supervise home health aides and collaborate with caregivers to ensure quality patient care.

Maintain accurate and timely clinical documentation.

Patient & Family Education

Educate patients, families, and caregivers on disease progression, symptom management, medications, and end-of-life care.

Teach caregivers how to safely perform tasks essential to patient care.

Provide emotional support and guidance to patients and families throughout the hospice experience.

Interdisciplinary Collaboration

Collaborate closely with physicians, social workers, chaplains, aides, and other members of the care team.

Communicate changes in patient condition and coordinate appropriate interventions.

Participate in interdisciplinary team meetings and care planning discussions.





Quality & Compliance

Adhere to hospice, state, federal, and regulatory requirements.

Follow infection prevention and control practices.

Participate in quality improvement and performance initiatives.

Ensure compliance with documentation and clinical practice standards.

Qualifications

Required Education

Graduate of an accredited Registered Nursing program or

Graduate of an accredited Licensed Practical Nursing program

Required Licensure

Current Georgia Registered Nurse (RN) or Licensed Practical Nurse (LPN) license

Valid Georgia driver's license

Reliable transportation and ability to travel throughout the service area

Required Experience

Minimum of two years of nursing experience

Experience providing compassionate care to patients and families experiencing serious illness or end-of-life care preferred

Strong assessment, communication, and critical-thinking skills

Physical Requirements

Ability to stand, walk, bend, lift, and assist patients as needed.

Ability to lift and carry up to 50 pounds occasionally.

Ability to work in a home care environment and travel between patient locations.

Schedule

Full-time, Monday through Friday, 8:00 AM – 5:00 PM

Occasional on-call responsibilities based on team rotation
