Advocate Health is seeking a Hospice Nurse Case Manager (RN/LPN) to provide skilled nursing care in the home hospice setting. You will coordinate interdisciplinary services, develop care plans, and support patients and families through the hospice journey.

The role includes admissions visits, ongoing assessments, care plan management, and collaboration with the care team to ensure high-quality, compliant hospice care. Full-time schedule, M-F with occasional on-call.
