Position: Full-Time Physical Therapist in Home Health, responsible for evaluating and treating patients within Warren & Claiborne Counties.

Responsibilities

- Perform patient evaluations, develop and implement treatment plans, and document care following policies and physician orders.
- Provide patient education, counseling, and coordinate with interdisciplinary team members.
- Maintain accurate documentation, ensure treatment goals are measurable and objective, and meet productivity standards.
- Communicate patient progress to physicians and team members, and adjust treatment plans as needed.

Qualifications & Skills

- Master’s degree in physical therapy from an accredited program.
- Minimum nine months of experience as a physical therapist; community/home health experience preferred.
- Current licensure to practice in the state; valid CPR certification.
- Valid driver’s license with ability to travel to various locations.
