pCenterWell Home Health currently seeks a licensed Occupational Therapist to provide in-home rehabilitation, helping patients regain strength, mobility and independence in their own homes.
You will work within a care team to implement individualized plans of care aligned with physician orders, and document patient progress for ongoing treatment.
