Job Summary
We are seeking a dedicated and dynamic Outreach Worker to join our team. This role involves engaging with community members, promoting our child and Family programs, and building strong relationships to support organizational goals. The ideal candidate will possess excellent communication skills, leadership qualities, and experience in project management and public speaking. Duties
- Provide direct outreach support to community members receiving Income Assistance and Adult In-Home Care services to promote stability, safety, and wellbeing.
- Work collaboratively with clients to identify needs and develop individualized support plans with clear, measurable short term and long-term goals.
- Collaborate and network closely with the Social Development Department to ensure coordinated, consistent, and culturally appropriate service delivery.
- Advocate on behalf of clients to access health, social, housing,
employment, and related services with external agencies and service providers.
- Identify, build, and maintain relationships with community programs, agencies, and informal support networks to enhance outreach services.
- Deliver health education outreach and support health promotion and prevention activities relevant to community needs.
- Prepare intake, assessment, and progress reports and maintain accurate, timely, and confidential client documentation.
- Monitor and evaluate service plans to ensure resources are used appropriately and supports remain effective and responsive.
- Coordinate and support the Adult In-Home Care Program, including homemaking services, file management, seasonal cleaning coordination, and service tracking.
