Job Summary

We are seeking a dedicated and dynamic Outreach Worker to join our team. This role involves engaging with community members, promoting our child and Family programs, and building strong relationships to support organizational goals. The ideal candidate will possess excellent communication skills, leadership qualities, and experience in project management and public speaking.

This position offers an opportunity to make a meaningful impact within the community while developing valuable skills in outreach, education, and program coordination.

Duties

- - Provide direct outreach support to community members receiving Income Assistance and Adult In-Home Care services to promote stability, safety, and wellbeing.

- Work collaboratively with clients to identify needs and develop individualized support plans with clear, measurable short term and long-term goals.

- Collaborate and network closely with the Social Development Department to ensure coordinated, consistent, and culturally appropriate service delivery.

- Advocate on behalf of clients to access health, social, housing, employment, and related services with external agencies and service providers.

- Identify, build, and maintain relationships with community programs, agencies, and informal support networks to enhance outreach services.

- Deliver health education outreach and support health promotion and prevention activities relevant to community needs.

- Prepare intake, assessment, and progress reports and maintain accurate, timely, and confidential client documentation.

- Monitor and evaluate service plans to ensure resources are used appropriately and supports remain effective and responsive.

- Coordinate and support the Adult In-Home Care Program, including homemaking services, file management, seasonal cleaning coordination, and service tracking.





- Support individuals, families, and community groups in identifying service gaps and advocating for needed programs and supports.

- Assist clients in navigating systems and understanding their rights, responsibilities, and available supports.

- Participate in case conferences, staff meetings, and interagency planning sessions as required.

- Maintain awareness of community trends, emerging needs, and systemic barriers affecting clients.

- Support community-based initiatives and activities that strengthen wellness, independence, and self sufficiency.

- Perform other duties related to Social Development programs and services as assigned by the Social Development Manager.

Qualifications

- - Post secondary education in Social Development, Human Services, or a related field, or equivalent experience.

- Experience providing outreach or community-based support services to individuals or families.

- Ability to support individuals facing social, health, or economic challenges.

- Strong interpersonal and communication skills.

- Ability to maintain confidentiality and handle sensitive information appropriately.

- Experience with client documentation, file management, and basic reporting.

- Knowledge of community resources and referral processes.

- Basic proficiency in Microsoft Word, email, and computer systems.

- Knowledge of Tl’azt’en Nation culture and community, or willingness to learn.

- Possess a valid BC Class 5 Drivers License and satisfactory Criminal Record check.
