/p /brh3What You’ll Do /h3 /brh3Lead Patient-Centered Hospice Care /h3 /brulliConduct comprehensive patient and family assessments /liliDevelop and manage individualized, comfort-focused care plans /liliDeliver skilled nursing care with a focus on pain and symptom management /li /ul /brh3Serve as the Care Coordinator /h3 /brulliAct as case manager, coordinating care across the interdisciplinary team /liliCollaborate with physicians, social workers, chaplains, and aides /liliMonitor patient condition and adjust care plans to maintain comfort /li /ul /brh3Support Patients Families /h3 /brulliProvide education, emotional support, and guidance throughout the journey /liliHelp families navigate complex decisions with empathy and clarity /liliBuild meaningful, trusting relationships in deeply personal moments /li /ul /brh3Drive Quality Compliance /h3 /brulliMaintain accurate, timely documentation /liliParticipate in IDT meetings and care planning /liliSupport quality and compliance standards /li /ul /brh3What You Bring /h3 /brulliActive RN license (or compact eligibility)
