Minimum Qualifications:
Education: One of the following:
Bachelor's degree in a human services field
Associate's degree in a human services field and two years of full‐time direct service experience
High school diploma/GED and four years of full‐time human services experience
Experience: Experience providing direct services to individuals with mental health needs, including counseling, case management, crisis intervention, and service coordination.
Certifications/Licenses:
No specific license required.
Must be able to travel within the community; a valid driver's license may be preferred depending on assignment.
Work Eligibility: Must be authorized to work in the U.S.
Work Location Requirement: Onsite / Community‐based field work (regular travel to consumer residences).

About the Role: This role functions as part of an interdisciplinary team supporting individuals in maintaining stable, independent living in the community. The position involves home visits, counseling, service coordination, crisis intervention, and advocacy to help consumers access medical, social, educational, vocational, and financial resources. Most duties are performed independently with general supervision.
