As a Hospice Registered Nurse (RN) , you play a vital role in delivering comfort, dignity, and compassionate support at the end of life.

Role OverviewAs a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.
You'll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes.
Through each visit, you bring not only clinical expertise, but also reassurance, clarity, and compassion.
What You'll DoLead Patient-Centered Hospice CareConduct comprehensive patient and family assessments
Develop and manage individualized, comfort-focused care plans
Deliver skilled nursing care with a focus on pain and symptom management
Serve as the Care CoordinatorAct as case manager, coordinating care across the interdisciplinary team
Collaborate with physicians, social workers, chaplains, and aides




Monitor patient condition and adjust care plans to maintain comfort
Support Patients & FamiliesProvide education, emotional support, and guidance throughout the journey
Help families navigate complex decisions with empathy and clarity
Build meaningful, trusting relationships in deeply personal moments
Drive Quality & ComplianceMaintain accurate, timely documentation
Participate in IDT meetings and care planning
Support quality and compliance standards
What You BringActive RN license (or compact eligibility)
Graduate of an accredited nursing program
Minimum 1 year of clinical nursing experience
Strong clinical judgment and compassionate care approach
Ability to manage a caseload independently
Excellent communication skills in sensitive situations
Basic EMR proficiency
Valid driver's license and reliable transportation

Strong pain and symptom management skills
Case management and interdisciplinary care experience
Ability to quickly build trust with patients and families
Experience with Homecare Homebase (HCHB)
