and any changes in condition Maintain accurate, timely documentation in the EMR Communicate effectively with the Physical Therapist and interdisciplinary care team Participate in case conferences and care coordination efforts Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated care Participate in team meetings, in-services, and ongoing training Uphold clinical standards and contribute to quality improvement efforts What Sets You Apart Strong patient-first mindset with a passion for rehabilitation Ability to motivate and engage patients throughout recovery Excellent communication and time management skills A compassionate, proactive approach aligned with our values: Trustworthy. What You Bring

Graduate of an accredited Physical Therapy Assistant (PTA) program Current PTA license or certification as required by state law 1+ year of experience preferred (home health experience a plus) Current CPR certification Valid driver's license, reliable transportation, and auto insurance Who We Are Founded in 2021, Vital Caring is a leading home health and hospice provider with 100+ locations nationwide.
