As an Occupational Therapist with Caring Hand Home Health Solutions, Inc., you will help patients regain independence, improve function, and safely navigate daily life - all from the comfort of their own home.

Please note that this role is per diem with a rate of $130 per visit.

Responsibilities
- Evaluate patients' functional abilities and home environments to identify needs and safety concerns
- Develop personalized treatment plans and communicate them to physicians and the care team
- Teach and retrain patients in daily living skills affected by illness or injury
- Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
- Maintain accurate clinical records and promptly report changes in patient condition
- Coordinate with the broader care team to ensure seamless, patient-centered care
- Educate family members and caregivers on the patient’s treatment approach
- Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
