ph3Job summary /h3pWe’re looking for an Independent Assessor and Prescriber to join our exciting ADHD service for 37.5 hours per week. This is a fantastic opportunity to make a difference from the outset and play an important role in helping us develop and deliver the service. /ph3Main duties of the job /h3pAs an Independent Assessor Prescriber you will work as part of the PCS ADHD service working across the interface between primary care, voluntary sector and secondary services, with the aim of improving the mental health and well-being of our communities. /ppYou will support individuals to move from surviving to thriving by helping people to feel in control of their health and wellbeing, by linking service users with health and social groups and pathways to address the impact of living with undiagnosed ADHD symptoms. /ppYou will be integrated as part of the primary care ADHD infrastructure, providing ADHD Assessment, Diagnosis and prescribing treatments. /ph3About us /h3pPrimary Care Sheffield (PCS) is a not-for-profit healthcare organisation delivering NHS services across Sheffield, including primary care, community services and mental health. We are committed to creating a supportive and inclusive environment where our people can develop, contribute ideas and make a meaningful difference to the patients and communities we serve. /ph3Job responsibilities /h3h3Clinical Practice /h3ulliDeliver structured diagnostic assessments (history, impairment, co-morbidity, risk) and provide clear feedback and written plans. /liliConduct structured follow-up reviews to monitor treatment response, adjust care plans and ensure safe, effective long-term management.



/liliWork with the pharmacy team to initiate and adjust medication within the local formulary and arrange baseline assessments and ongoing physical health monitoring. /liliConduct brief titration contacts until the patient is stable. /liliComplete safe shared-care transfers to primary care using the Electronic Prescription Service and relevant templates. /liliDocument clinical activity promptly and accurately, produce high-quality letters on the day, and maintain accurate and auditable records. /liliTo maintain high standard of care in all areas of practice in accordance with Local and National standards. /liliMaintain and develop professional knowledge, skills and expertise, acting at all times in accordance with professional standards and ensuring practice remains responsive to changing care needs. /liliWork within the scope of professional competence and autonomy, recognising when guidance, support or escalation is required. /li /ulh3Leadership and Service Development /h3ulliPresent complex primary care cases at the weekly MDT and contribute to audit, incident learning and quality improvement. /liliSupport the induction of colleagues, participate in case discussions and provide day-to-day coaching, while upholding safeguarding, consent and capacity, and information governance requirements at all times. /liliSupport the effective functioning of the service through leadership tasks as required and use leadership skills to support the wider multi-professional team. /liliContribute to the development of procedures and undertake delegated tasks as required. /liliSupply the necessary information of activity outcomes (and other KPI data) as agreed with the Service Team Manager and Head of Service.



/liliUse IT skills appropriate to the needs of the post and contribute to the use of new IT tools and reporting systems, including the emerging use of AI. /liliExercise professional judgement to promote safety when managing complex or unpredictable situations, demonstrating effective teamworking, resilience and determination, and ensuring concerns are appropriately escalated. /li /ulpThis job description is not exhaustive.
