Role Overview As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.
What You'll Do Lead Patient-Centered Hospice Care Conduct comprehensive patient and family assessments
Deliver skilled nursing care with a focus on pain and symptom management
Serve as the Care Coordinator Act as case manager, coordinating care across the interdisciplinary team
Support Patients & Families Provide education, emotional support, and guidance throughout the journey
Help families navigate complex decisions with empathy and clarity
Drive Quality & Compliance Maintain accurate, timely documentation
Participate in IDT meetings and care planning
Support quality and compliance standards
What You Bring Active RN license (or compact eligibility)
Graduate of an accredited nursing program
Minimum 1 year of clinical nursing experience
Valid driver's license and reliable transportation
What Sets You Apart Experience in hospice, palliative care, or home health
Case management and interdisciplinary care experience
Ability to quickly build trust with patients and families
Experience with Homecare Homebase (HCHB)
