The mission is to take care and treat patients at home with expertise, ethics and devotion, developing services increasingly closer to the patient’s needs. /p h3Role Overview /h3 pContribute to the delivery of a suite of services to patients at home, including installation of medical devices and training/coaching activities, to support chronic disease management in accordance with medical prescriptions and local regulations. /p h3Key Responsibilities /h3 ul liAct as the primary point of reference for the patient, family (caregiver) and the multidisciplinary team within the Central Operational Office. /li liFacilitate and optimize relationships and communication with General Practitioners, Health Districts and territorial operators. /li liMonitor the correct application of Individual Care Plans, ensuring clinical appropriateness and alignment with predetermined care objectives,



in collaboration with the Health Coordinator. /li liAssign care activities to operators, handle clinical‑health communications and interface with ATS applications. /li liEnsure continuous and proactive monitoring of clinical outcomes and health data using the electronic clinical chart and available digital tools, managing the preparation and verification of documentation and periodic reports. /li liSupport operational management (e.g., logistics of medical materials, archiving), contribute to onboarding, training and education of new healthcare operators in the area, and guarantee correct use of medical devices provided by Medicasa. /li /ul h3Required Qualifications /h3 ul liDiploma in Nursing with at least 5 years of experience. /li liCompetence in installing medical devices. /li liExcellent communication and relational skills with patients (pedagogy, ability to explain, etc.). /li /ul h3Equal Opportunity Statement /h3 pWe welcome and consider applications from all qualified applicants, regardless of their background. /p /p #J-18808-Ljbffr
