Total Life is seeking nurse practitioners for an in-person contract role in the Lodi-Sacramento area to lead longevity days at partner senior living communities. You'll open with an educational talk and then conduct ***** minute longevity visits built on a comprehensive geriatric assessment.
You'll be trained by our Chief Medical Officer and will coordinate with primary care providers, with Medicare documentation to be completed in our EHR.
