Founded in 2011, our agency offers trauma-informed, low barrier programs that support young people in exiting the streets and achieving stability. If you have the skills, passion, and collaborative spirit, join our growing team /p h3ABOUT THE ROLE /h3 pThe Case Management Supervisor, Access Center is responsible for the oversight of daily operations of case management activities, including supervising the Member Services team which consists of Case Managers, Problem Solving Specialists, and Housing Navigators. They will ensure that all Access Center case management functions are performed accurately and efficiently and in compliance with all applicable state and federal regulatory requirements. This position requires extensive coordination within multiple internal SPY Programs and external systems that include relationships with the Housing Authority, DCFS, DMH, LAHSA, St. Joseph’s Center, community partners and resources, landlords, and property owners and managers. /p h3WHAT YOU’LL DO /h3 ul liDevelop, implement and supervise case management workflows in compliance with all applicable state and federal regulatory requirements /li liSupport the day-to-day operations and programing of the Access Center, including site monitoring /li liSupervise direct service staff in providing case management, housing navigation, and problem solving /li liWork closely with the Access Center Manager to administer problem solving funds and financial assistance /li liReview case management notes and service plans, to ensure that all members served are receiving high quality care /li liMonitor and provide continuous quality improvement of member charts, data collection and analysis, monthly reports, and program administration duties in a timely fashion /li liEnsure that program outcomes are being met and that performance and quality improvement strategies are being embedded in the work /li liDeveloping a high performing department culture and staff. This includes setting the standard for staff/peers and motivating employees to maximize organizational goals and objectives. /li liSetting goals and planning, assigning, and directing work consistent with said goals. This includes responding to employees’ needs, ensuring they have the necessary resources to do their work.



/li liAppraising performance, rewarding and disciplining employees, addressing complaints and resolving issues. This includes providing regular and effective feedback to employees and completing timely and objective performance reviews. /li liProvide individual and group supervision to staff /li liFacilitate case assignment, case conferencing, and other relevant care coordination or treatment planning meetings /li liAct as a preceptor for MSW/MFT interns /li liProvide mediation and crisis intervention to youth as needed /li liAdvocate by attending external community meetings with government agencies and community groups /li liWork within established legal and ethical guidelines /li liAbility to work nights/weekends as needed /li liOther duties as assigned to support the mission of the organization. /li /ul pRequirements /p h3WHAT YOU’LL NEED /h3 ul liBachelor's Degree in Social Work, Psychology, Sociology or related field OR comparable experience /li li3-4 years’ of case management or related experience working with unhoused and at-risk TAY is brequired. /b /li li1-2 years of experience working in a drop-in center, access center, or access point is bpreferred. /b /li li1-2 years of experience supervising direct service staff. /li liKnowledge and experience working with co-occurring populations (chronic health conditions, mental health and substance abuse) preferred /li liKnowledge and experience working with LA-based housing resources, partners, and programs. /li liBest practice models including: Housing First, Positive Youth Development, Authentic Youth Engagement, Motivational Interviewing, Harm Reduction, Trauma-Informed Care, Critical Time Intervention. /li liSelf-Starter skill set, requiring the ability to develop relationships, build rapport and trust with members, community partners, volunteers, and staff. /li liStrong verbal and written communication skills, including organizational, detail and interpersonal skills. /li liAbility to uphold Access Center policy and procedure with members, staff, volunteers, and community partners /li liAbility to work effectively with people of diverse races, ethnicities, sexual orientations, and gender identities in a multicultural environment. /li liWorks cooperatively with individuals, groups and organizations with diverse mission, composition, function, capacity and geographic location. /li liCultural Responsiveness /li liA commitment to and passion for the mission, vision,



values and culture of SPY /li liA self-starter who is prepared to drive innovative solutions for an emerging organization /li liAn ability to work in a fast-paced environment that requires effective collaboration with a variety of organizations and individuals /li liProficient in Microsoft Office and Google Drive /li liMust have reliable transportation, valid California driver's license and proof of current insurance. /li /ul h3WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS /h3 ul liProlonged periods of sitting at a desk and working on a computer. /li liChanges in the environment, such as the office or outdoors. /li liPhysical effort/lifting, such as sedentary- up to 15-20 pounds at times. /li /ul h3SPECIAL CONDITIONS /h3 h3MANDATED REPORTING REQUIREMENTS /h3 pAll SPY employees are mandated reporters and are legally required to report any reasonable suspicion of child, elder, or dependent adult abuse or neglect. /p h3COVID-19 Vaccination Requirements /h3 pEffective August 9, 2021, the State Health Officer has mandated that all workers, paid or unpaid, in homeless services organizations must provide proof of vaccination.
