Our Mission:
To empower individuals with behavioral health conditions, substance use disorders, and/or developmental disabilities to achieve the lives of their choosing in the community.
Schedule:
8am- 3:30 pm - (must be able to work 1pm- 9pm 2 nights/week and every other weekend)
Summary of Job Description:
Provide direct oversight of Member residential living arrangements to ensure safety, cleanliness, and overall environment of recovery. Support and document Member self-administration of medications.
Specific Responsibilities:
- Roving and inspection of residences and Member ADL support as needed.
- Support with facilities and medication management activities, including (but not limited to) the following:
- Disaster Drills
- Residential Checklist
- Medication Room Checklist
- Van Checklist




- Implement and document Member interventions and progress relative to Member Treatment Plan as it pertains to medication and ADL compliance.
- Complete Incident Reports in accordance with Project Transition/ PCS Mental Health’s Policy and Process.
- Complete progress notes for any interactions with Members and document in the Electronic Health Record (EHR) (DAP format) by the end of your covered shift.
- Facilitate/ Co-facilitate all groups as assigned and complete documentation within 24 hours of facilitating.
- Support with transporting Members per the directive of the Program.
- Support with observation of Member alcohol and drug screenings as assigned and document observations by the end of the covered shift (if applicable).
- Support Members with activities of daily living including (but not limited to) the following:
- Daily oversight and coaching of daily living activities.
- Observing Member self-administration of medication.
