Role Impact Manifesto As a Care Coordinator, you serve as the link between science, service, and each member's journey. You bring together the expertise of physicians, nutritionists, coaches, and other medical professionals into a unified care plan. Your leadership makes sure every member feels seen, heard, and supported. You bring calm to chaos, momentum to doubt, and elevate wellness into a premium experience. What You'll Do As a Care Coordinator, you will oversee and manage the entire member journey from onboarding through diagnostics, plan development, care implementation, and renewal planning. You will ensure each step is personalized, accurate, and timely. You will serve as the primary point of contact for your assigned members, fostering trust through effective communication, scheduling,



proactive outreach, and ongoing support. Your daily efforts have a significant impact on both clinical quality and emotional engagement.
You will collaborate with multiple teams to ensure care plans are executed and followed up on. You will track milestones, identify gaps, and promote teamwork across departments. From verifying diagnostic readiness to coordinating plan meetings and preparing mid-year reviews, you will ensure that every member's experience reflects the Fountain Life standard of accuracy and excellence.
