You will be the first point of contact for young people and their families making use of the service. This role is about creating excellent rapport, identifying the support the young person needs and collating the initial information. Reporting to the Centre Manager, your responsibilities will include: Assisting young people to access services and providing coordination of care to meet individual needs; Developing a clear understanding of your clients through case formulation and assessment; Collaborating with the team to implement access to information and a range of culturally-sensitive activities for youth; Conducting intake assessments and initial risk assessments; Coordinating services with other allied health services in the region; Providing therapeutic
