Garantire l'educazione sanitaria del Paziente; favorire e sostenere la partecipazione attiva nella determinazione delle decisioni terapeutiche.* Attuare interventi infermieristici specifici, autonomi e complementari di natura tecnica, relazionale ed educativi in linea con le attività previste dal PAI;* Garantire le attività assistenziali orientate alle esigenze dei pazienti considerando età, genere, condizioni di salute, fragilità fisiche/psichiche e tenendo conto delle specificità religiose etniche e linguistiche;* Garantire l'educazione terapeutica all'Assistito e al care giver;* Agire secondo le Linee Guide Internazionale e le Procedure definite da Medicasa;* Garantire il corretto utilizzo dei materiali e dispositivi messi a disposizione da Medicasa e corretto stoccaggio a casa dell'assistito* Garantire l'identificazione e la pianificazione degli obiettivi assistenziali e verifica il monitoraggio degli outcome clinici;* Garantire la corretta tenuta della documentazione sanitaria ed amministrativa utilizzando la Cartella Clinica informatizzata.* Partecipare alle riunioni di Team organizzate in Centrale Operativa;* Garantire la redazione delle relazioni sanitarie periodiche e se richieste dall'area sanitaria;* Partecipare alle riunioni indette dalla centrale operativa, dai distretti sanitari;**Our Differences make our Performance** At Air Liquide, we are committed to build a diverse and inclusive workplace that embraces the diversity of our employees, our customers, patients, community stakeholders and cultures across the world.We welcome and consider applications from all qualified applicants, regardless of their background.
